Fit (sick) notes update

Effective immediately, our practice will no longer accept requests for Fit (sick) notes via telephone or in person.

It is no longer necessary to book an appointment with a GP to discuss these requests. Instead, all Fit note requests should be submitted through https://accurx.nhs.uk/patient-initiated/p84065/questions and the sick note will be sent to you via SMS.

Please make your own arrangements to print it out if needed.
